Whether you’re interested in day training, residential support, or employment services, our Enrollment Team is here to guide you every step of the way. We’ll answer your questions, help you explore your options, and support you in finding the services that best meet your needs and goals.

To begin the enrollment process, please complete our Enrollment Interest Form, which helps us get to know you and how we can support your journey.

Once the form is completed, someone from our intake form will be in touch about our full intake paperwork required.

As part of the intake process, you’ll be asked to provide important information that helps us personalize your experience, such as:

• Your contact and demographic information
• Details about your disability and support needs
• Current services or funding sources (such as Medicaid Waiver, private pay, or other insurance)
• The types of programs or services you’re interested in
• Your availability and location preferences
• Any goals or areas of growth you’d like to focus on

Once we receive your completed forms, a member of our Enrollment Team will reach out to walk you through next steps and answer any questions you may have.
